The date of 25 May 2018 is the effective day of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (further referred to as the 'GDPR'). Satisfying the disclosure requirements, under the GDPR, art. 13, items 1 and 2, Platige Image S.A. Capital Group hereby informs that:

1. your personal data are administered by the capital group of Platige Image S.A. with the registered office in Warsaw (02-634), Poland, at the address of ul. Racławicka 99, entered into the Register of Entrepreneurs of the National Court Register under number KRS:0000389414, and having Tax Identification Number NIP: 5242014184;

2. all correspondence regarding the issues of personal data protection must be addressed at the following e-mail: rodo@platige.com;

3. your personal data will be processed for the following purposes:

a. performance of the cooperation agreement in compliance with the GDPR, art. 6, item 1(b)

b. satisfaction of the legal obligations on the part of the personal data controller in compliance with the GDPR, art. 6, item 1(c)

c. provision of additional services or benefits, if you have agreed to have your personal data processed for this purpose, in compliance with the GDPR, art. 6, item 1(a).

4. your personal data will be stored for the time necessary for the performance of the cooperation agreement with Platige Image S.A. or other entities of Platige Image S.A. Capital Group and for the satisfaction of the obligations arising there from, as well as for the pursuit of and defence against claims based on the general limitation periods for claims and extended by five years or upon your withdrawal of the consent in the scope specified by you without affecting the legality of the data processing which took place before the;

5. you have the right to access, correct, delete and transfer your personal data, to limit the processing scope, to raise objections and to withdraw your consent for processing at any time without affecting the legality of the processing which took place on the grounds of the consent given before the withdrawal;

6. you have the right to submit a complaint to the President of the Personal Data Protection Office if in your opinion your personal data are processed in violation of the GDPR provisions;

7. provision of your personal data is obligatory and inevitable for the performance of the cooperation agreement with Platige Image S.A. or other entities from Platige Image S.A. Capital Group, although in the scope specified in point 4 above it is voluntary but necessary if you wish to be included in the sports and/or medical cover programme;

8. it is in your interests to update your personal data promptly.
